To get a copy of a blank application, program guide and related forms, just click on the "Forms/Supplies" tab in the upper right corner of the ForemostSTAR home page.

This tab will bring up an easy-to-use window that lets you access the forms in PDF format or order supplies directly from the Foremost Supply Department. You can also consult program guides and related materials for your individual state.

Don't forget that you can quote and book most types of specialty insurance online with ForemostSTAR, which is secure and safe for all business transactions. Policy documents generated can also be printed on paper or saved for future reference.

To retrieve policy documents for existing customers, use the Electronic Document Retrieval (EDR) service on ForemostSTAR. Just click on "Get Policy Documents" at the top of the ForemostSTAR home page. This full-feature system gives you immediate access to a wide variety of customer records, including DEC pages, billing records, claims status reports and more.
